Community-based dialysis clinic supporting patients from all hospital renal programs * access to permanent, temporary and respite hemodialysis treatments
Care is provided on site by experienced nephrology clinicians and monitored in collaboration with each patient's hospital team (i.e. nephrologist and allied health)
