Application:

To book an appointment to complete your application, you must first apply to the Community Homes for Opportunity (CHO) program. Applications can be submitted by calling 905-686-0159 or by completing the Intake Assessment form online. After your application has been submitted, you can then schedule an appointment to continue the process.
